Your First Office Visit
We want to make your first visit to American Indian Health Service as pleasant and comfortable as possible. Here are a few tips that will help us serve you better.
What to Expect
On your first visit, we will ask you for some information about yourself and your insurance carrier if you have one. We will want to make a copy of your tribal enrollment card, insurance card, picture ID and proof of income (if applying for sliding fee scale discount). You will also be asked to complete new patient forms including patient information and medical history. You may want to save some time by printing the forms, filling them out and bringing them with you. All forms can be read in Acrobat Reader.
What to Bring With You
In order for your physician to better serve you and provide the best possible care, please bring the following items to your appointment:
- ^If you have insurance, please bring your Health Plan Insurance card
- ^Any necessary or related medical records
- ^A list of the medications you are taking, including dosage
- ^A list of questions or health concerns you would like to ask your healthcare provider
