Reducing The Burden On Local Hospitals

Our Level III Patient Centered Medical Home increases health outcomes for patients and reduces the need for non-critical visits to area hospitals and emergency rooms. We provide the community with same-day access to our primary care physicians, specialists and counselors and after-hours telephone access. Our dedicated medical staff track and coordinate patient care between physicians and specialists and outside care such as pharmacies and hospitals.

We employ have a staff nutritionist who provides healthy diet counseling and diabetes education.
